Registration and general information on consultation hours
Please provide us with the following documents in advance (during or immediately after registration) by e-mail, post or fax:
- informative doctor's letters on your question and any previous genetic findings
These documents are used to prepare and plan your appointment. Missing documents will delay the genetic clarification and may prolong your appointment with us considerably.
Registration for all consultation hours
Appointments daily by appointment at the Medical Care Center (MVZ) of the University Hospital Cologne or at the Institute's outpatient clinic.

Please bring the following with you to your first appointment:
- if you have statutory health insurance and are attending the genetic outpatient clinic (in the university outpatient clinic), please bring a yellow referral form according to sample 6 (in the MVZ Humangenetik, however, the electronic health insurance card is sufficient)
- when presenting patients with statutory health insurance in our special consultation hours in accordance with 116b / ASV, a yellow referral slip according to sample 6 (if you have any questions, please contact Ms. Gniostko)
- the yellow or green examination booklet when presenting children
- new medical reports or findings relating to your question (if you have not already sent these to us as part of the registration process)
- possibly an interpreter or a family member as translator
